This plan is a Limited Benefit Health Insurance Plan. Plans are underwritten by Starr Indemnity & Liability Company (Form No. AH-70001CN) or Companion Life Insurance Company (Form No. MMP2250) offered through a paid association membership and vary by availability, vendor and state of residence.

It is NOT MAJOR MEDICAL INSURANCE and is not meant to replace catastrophic health insurance or major medical coverage. Limited medical benefit plans previously available through Cinergy Health are underwritten by American Medical and Life Insurance Company (Form No. GRP LM 2007). The discount dental and prescription programs are provided by Patriot Health Florida, Inc. (except in CA), a discount medical plan organization. These programs provide discounts at certain health care providers for medical services and do not make payments directly to the providers of medical services. The member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with Patriot Health Florida, Inc., located at 160 Eileen Way, Syosset, New York 11791.
